What fixed it for me was exiting iTunes, moving everything out of the iTunes Media folder on the computer, deleting all music on the phone in Settings, connecting the phone again and syncing both to end up with an empty Library, exit iTunes on the computer, move everything back to the iTunes Media folder and then open iTunes again with the phone connected.
